Prenatal Care Coordination

The Prenatal Care Coordination (PNCC) Program is a Medicaid benefit that helps pregnant women get the support and services they need to have a healthy baby.

The program provides:

  • Health care personal support
  • Information on good eating habits and health practices
  • Information and help finding services in the community

Participants meet with the care coordinator monthly to discuss  the participant’s identified needs and to talk about any worries that they have regarding their pregnancy and getting ready for their new baby.  Education, resources and referrals are also provided along with an opportunity to talk about what is going on in the participants live.
